Scheduling an appointment with iThrive Health & Wellness Health Clinic is easy and convenient. Our team is here to help you find a time that works for you. We currently are seeing ages 12 and up.
Please have the new patient packet, questionnaires, and consents completed prior to your appointment time. This enables us to cover a large amount of information in a short time and better serve your needs. Appointments will have to be rescheduled if not completed. Thank you for your understanding!
We offer gift cards which can be purchased in any amount when coming into the office or calling. We can provide a physical copy or an email copy based on your preference.
Regarding refunds:
Gift cards are not returnable. Once they are purchased, all sales are considered final.
According to the law, if the customer requests a refund, a company is required to redeem a gift card or certificate for its value or replace it with a new gift card at no cost.
While we do not accept insurance for office visits, you may print a copy of your superbill from your patient portal to submit to your insurance for possible reimbursement.
Each person's reimbursement is different based upon their insurance policy. Most all labs can be done through insurance, however some of our more advanced functional tests may require a co-pay or may not be covered at all. This can be discussed during your visit. We strive to keep diagnostics as cost effective as possible!
