There are many different tests that can be done to evaluate thyroid. The most two common test that are done in primary care are TSH and T4.
It is important to know what the significance of each test and what they mean.
TSH is simply a signal from the pituitary to the thyroid gland. It has nothing to do with how much thyroid hormone the gland is producing.
Approximately 80% of T4 is made by the thyroid gland, however this is the inactive form of thyroid hormone and must be converted to the active T3.
Under times of stress, caloric restriction, intense exercise, infections, and in the case of some nutrient deficiencies and taking certain medications, T4 thyroid hormone is preferentially converted over to reverse T3 which actually blocks the active T3 thyroid hormone from entering the cells. This can also create hypothyroid symptoms.
